A shocking friendly fire incident has raised serious questions about coordination among allied forces in the Middle East. Reports now suggest that three US fighter jets were accidentally shot down by a Kuwaiti F-18 during intense combat operations over Kuwait.

The incident, described as a “blue-on-blue” episode, reportedly occurred on Monday when a Kuwaiti fighter pilot mistakenly launched three missiles at American aircraft during what officials called “active combat” conditions.

All three US jets, identified as F-15E Strike Eagles, went down over Kuwait. Fortunately, the crews ejected safely and survived.

According to reports, at least one F/A-18 Hornet operated by Kuwait fired the missiles that struck the American jets. A US official reportedly confirmed that the Kuwaiti pilot mistakenly engaged the three aircraft amid confusion caused by incoming Iranian drones.

The situation unfolded as multiple Iranian drones reportedly entered Kuwaiti airspace. One drone struck a military base, killing six American personnel, a day before the fighter jet incident. Kuwaiti forces were on high alert at the time of the shootdown.

The downed jets were not flying over Iran but over Kuwaiti territory, making the episode particularly sensitive.

What Did US Central Command Say?

The United States Central Command (CENTCOM) confirmed that the incident happened during “active combat,” involving “attacks from Iranian aircraft, ballistic missiles, and drones”.

Chairman of the Joint Chiefs of Staff Gen Dan Caine acknowledged the “loss” of the three F-15Es during a Pentagon briefing. He stated that US officials quickly determined the incident “was not from hostile enemy fire” but declined to provide further operational details.

CENTCOM commander Adm Brad Cooper later said that nearly 200 fighter aircraft have participated in ongoing operations, conducting around 2,000 strikes since the campaign began.

Friendly Fire Incident of Kuwait and US Jets: Why Is This Incident Significant?

This marks the first loss of an American aircraft since Operation Epic Fury began on February 28. The operation followed joint US-Israeli airstrikes on Iran that killed Supreme Leader Ali Khamenei. Iran responded with missile and drone attacks targeting US and allied positions across the Gulf region.

The F-15E Downing also represents the second known friendly fire incident involving US fighter jets in the Middle East in the past 15 months.

Military analysts describe the situation as unusual and troubling.

An ex-Air Force fighter pilot told Air & Space Forces Magazine the incident is “perplexing,” given that allied pilots are trained to follow procedures to prevent such dangerous mistakes.

“If you’re flying air defense missions, the first thing you do is interrogate using your transponder,” the former pilot told the magazine.

American pilots, he said, broadcast an Identification Friend or Foe code that identifies them as friendly to allied forces.

“It’s pretty hard to mistake an F-15E for an Iranian aircraft, and particularly if they’re not maneuvering in any kind of aggressive fashion,” the former pilot told the magazine.

Even if the F-15s were flying without transponders turned on, “that’s not a basis to engage those aircraft,” the former pilot said.

“They have to be demonstrating some hostile intent. If they’re just flying, that’s not hostile intent,” he added.

What Happened in Kuwait Airspace That Day?

Kuwait’s Ministry of Defense stated that it “confronted several hostile aerial targets” around dawn on the day of the incident. A later statement confirmed that several US aircraft had crashed.

Videos circulating online reportedly showed one F-15 spiraling downward with flames trailing from its rear section.

The incident occurred at a time when Kuwaiti defenses were on edge. A day earlier, six US Army soldiers were killed in an Iranian drone strike on a tactical operations center at Shuaiba port.

With regional tensions rising and aerial threats increasing, the risk of miscalculation appears to have grown significantly.

Investigators are now reviewing flight data, radar records, and communications logs to determine exactly how the misidentification occurred. Officials have not clarified whether the US aircraft had their IFF systems activated at the time.

As air operations continue across the Gulf, the incident underscores the dangers of high-intensity warfare involving multiple allied forces operating in congested airspace.
